Medication Therapy Adherence Clinic (MTAC) Module

MTAC is an Ambulatory Care Services conducted by the chief pharmacist with

emphasis on medical management of patients to improve quality, safety

and cost effectiveness in patient care.

The selection of patients for MTAC is through the patient's medical records or

Clinical Information System and reference from other healthcare providers.

This module involves the following activities:

· Review patient medication records

· Development of clinical records by type of MTAC

· Confirmation of patient appointments

· Continuous monitoring of patient’s condition (care plan)

· Subsequent references for other healthcare

 

Ward Supply Module

This module includes the following components:

· Receive on-line prescription

· Prescription screening, including drug alerts (such as drug overdose and

allergy), drug interaction, duplication

· Approve and confirm the prescription

· Generation of labels, preparation and filling, and dispensing to patients in

the ward as well as discharge patients

· Barcode labeling for security and safety aspects

· Detect and prevent the supply of multiple medications

· Order online and supply medicines to the ward from the common stock,

including floor, stock of drugs, psychotropic drugs and Dangerous Drugs

· Drug inventory and cost of drugs function for Pharmacy and Ward

· Computation of drug charges using approved fee and full-pay

patient fee

 

Ward Pharmacy Module

This module covers all clinical activities at ward level that are taken up by the

chief pharmacist:

· Retrieve the patient's medical history

· Provide patient medication record and advice patients and hospital

staff

· Monitor patient on-going progress through the "care plan"

· Prepare discharge reports for referral to hospital or health clinic so that

patients can receive necessary follow-up treatment

· Medication Reconciliation (MR) – Review, check, obtain and record

information about medications or alternative medicine to be prescribed for the

patient

· Discharge/Bedside counseling and dispensing - Supply and counseling on

drugs to be given to patients in the ward or during discharge

· Generate daily activity reports for ward pharmacy and any intervention

which has been carried out

Medication Counseling Module

This module covers all aspects of the activities of individual patient counseling,

Medication Therapy Clinic Adherence (MTAC), and group counseling. It involves:

· Reference the patient's history from the clinical system

· Update patient medication record, set an appointment for counseling, and

advisory services

· Monitor patient on-going progress

· Schedule a group counseling program

· Accept/Receive a referral for counseling

· Make a reference for counseling

 

Ward and Outpatient Prescription (Order Management) Module

This module allows the prescriber to order drugs on-line and send to Pharmacy

application for processing. This module contains the following components:

· Drug orders

Outpatient

Inpatient

· Drug alerts (interaction, allergy, adverse side effect, overdose, therapeutic

duplication) needed to assess the status of medication orders being

placed

· Access and interface to drug information database (subscribed external

database)

· Patient’s medication records history (medication history) to be provided

Medication Error (ME) Reporting and Adverse Drug Reaction (ADR)

Monitoring Module

This module contains the following two activities:

a. Medication Error (ME) Reporting Module

b. Adverse Drug Reaction (ADR) Monitoring Module

This module is used by all healthcare providers to report their incidents on ME

and ADR. ME and ADR reports can only be accessed by superusers or

authorized officers for the reports (Authorised personnel).

A. Medication Error (ME) Reporting Module

· Online delivery of medication error report

· Review reports submitted. Contact the reporters for more information if

necessary.

· Send reports to Medication Safety Center (MedSC), either online or

manually

· Generate database and reports for Medication Error (ME),

 Adverse Drug Reaction (ADR) Monitoring Module

· Online delivery of potential ARD incident report

· Record the ADR incident and send it online to authorities

· Build the database for ADR cases

 

Data Mining and Reporting Module

· Provide other reports, statistics and statistical analysis according to the

needs of each module.
